Go look up the definition of "standard enthalpy of formation" or "standard heat of formation" and all you need is there.

The question was
"what three things are needed for a heat of reaction to be a heat of formation?". 

OP said they had looked for an answer online but given how easy it is to Google the answer I doubt they did anything before posting here.  I gave them a nudge about how to find the answers.
