Why does the voltage decreases as the electrochemical cell works and the concentration of ions begins to change both the sides ?

I know the answer based on the equation but what is the basis of the equation ?

Thermodynamics. System tries to go the equilibrium which is the least energy state. As long as there is potential difference there is an energy available. If you try to use thermodynamics to mathematically describe the Gibbs energy of the redox system you get the Nernst equation.
