polymer reaction

I need the monomer(s) used to prepare the polymer in the attached picture; i am not even sure where to begin

Bottom part - where the 'poly' chain is drawn - is obviously product of the vinyl polymerization. Drawing monomer is not that difficult - just put double bond where it was before polymerization.

start with the name of the monomer - ethyl acrylate
