HOCN and HNCO conjugate bases

Can someone please explain to me why HOCN and HNCO are two dfferent compunds but their conjugate bases are the same? Can you please explain with structures?

I'm guessing you're talking about cyanic and isocyanic acid. These two are tautomers. My guess for the conjugate-acid base is in the attached picture.
