For the 2nd question, what does the diagram show?

4a is Oxygen , Carbon and Hydrogen I know that for 4b I need to count the dot and crosses?

How do ionic and covalent bonds differ?  What are the driving forces behind them both?

No, not the dots and crosses - these represent electrons.
