hybridization state

i wanna make sure if i am right coz my current syllabus doesn't cover hybridization involving d orbital
P in PCl5 > sp3d
Si in SiH4 > sp3
Cl in HOCl4 > sp3
i m not sure about the case of HOCl4 , there are 1 hybridized s , 3 hybridized p and 3 unhybridized d orbitals form 3 pi bonds with 3 sp2 O , do i get it right ??

Are you sure it's HOCl4 and not HClO4?

The others seem correct.
