Chemistry Lab Dilemma

Hi, I'm a currently an undergraduate physics student and I'm having real trouble with chemistry lab reports.
My question is that what are the key points of writing report, some details, results and discussion etc. My average score is 60-70 out of 100.
I'm wondering if you could show me how to write report with my current lab report assignment which I'm having real trouble with, I really need help on that one. (report sheets are on the google photos link), I don't know how to do those calculations, if you could help me how, I'd really appreciate that. Thanks
All of those data points are should be in half (except the ones with handwriting).
